Forms — Three Rivers Promise

Thank you so much for being interested in the Three Rivers Promise. We are currently in the initial fundraising phase of the promise. Scholarship funds are not yet available, and we are not accepting applications at this time. We will issue a press release when we are ready to start accepting applications.

Application for The Three Rivers Promise Scholarship

Students must apply by submitting an application during their senior year. Both online and printed applications are available. See the Apply page of this website for more information about eligibility and to submit an online application or see the high school guidance office for a printed application.

Three Rivers Promise Scholarship Acceptance Form

This form is submitted after the Application once a student has been accepted to the college, university, or trade school that they want to attend. This form allows the Three Rivers Promise to complete the payment process and allows the school to share academic progress.

This form is also used to transfer the Three Rivers Promise to another qualified education institution.

The completed form must be mailed to the Three Rivers Promise at PO Box 405, Three Rivers, MI 49093 or dropped off at the student’s high school guidance office.
